Piles are extremely uncomfortable, painful and embarrassing problems that affect several people of the world annually. Though the condition is not fatal, it is very disconcerting and may make the person lose interest in all activity. It is necessary to treat piles as soon as possible because it may complicate into grotesque proportions, leading to much shame to the sufferer.

Piles, or hemorrhoids as they are medically termed, is such a prevalent condition that almost half of the teenagers in most temperate countries will have suffered from the condition to a little or severe extent. Despite that, piles can occur in people of all ages and the condition becomes more and more serious as the age advances.

Definition of Hemorrhoids / Piles (Arsha Roga)


Hemorrhoids or piles are defined as the protrusion of varicose veins from the rectal mucus membrane in the rectal canal. This causes a swelling-like formation in the rectal area, which can grow to quite large sizes if not checked in time.

The Sanskrit name for piles is arsha roga. Arsha is the Sanskrit term for any sharp instrument and roga means disease. Hence arsha roga means a disease that is characterized by sharp pains.

Types of Hemorrhoids / Piles (Arsha Roga)

There are two main types of piles – internal hemorrhoids or piles and external hemorrhoids or piles.

Internal piles are formed above the dentate line of the anus. They are not visible externally. Internal piles are painless, but they can cause bleeding if there is a rectal prolapse. This occurs when the person exerts pressure on the anal aperture during defecation. When this occurs, the piles are called as bleeding piles. Bleeding piles are known as rakta arsha roga in Sanskrit.

External hemorrhoids or piles are formed below the dentate line of the anus. They are visible externally. These piles are very painful. There is bleeding but the blood is in the form of clots. The area near the anal opening gets inflamed and therefore the skin around the anus also starts to pain. External hemorrhoids or piles are also called as dry piles as the blood expelled in them is dried out into clots.

In most people suffering from piles, there may be symptoms of both internal hemorrhoids and external hemorrhoids observed together.

Causes of Hemorrhoids / Piles (Arsha Roga)

Ayurveda suggests that piles are caused due to the following factors:-

  • Low digestive fire
  • Vitiation of any or all of the three doshas, i.e. vata, pitta and kapha

The vitiation of apana vayu is especially linked with the formation of piles. Apanavayu is the vata dosha that is responsible for the movements of the excretory organs of the body such as the bladder, rectum and the uterus.

The following factors can bring about vitiation of the doshas that is responsible for piles:-

Difficulty in expulsion of stools from the rectum

Regular constipation

Varicosity of veins in the rectal area

Continuous sitting, such as while traveling

Sitting regularly on hard areas such as school desks

Continuous lifting heavy loads can exert the rectum

Pregnancy, since the fetus may be putting pressure on the veins leading to the rectum

Obesity, which causes pressure on the lower half of the body

Cancers of the pelvis or the bowels can exert pressure on the rectal area

Symptoms of Hemorrhoids / Piles (Arsha Roga)

The common symptoms of internal and external hemorrhoids are as follows:-

The initial symptoms of hemorrhoids are itching around the anal area.

The itching is usually accompanied with pain, especially when passing stools.

Formation of a lump occurs in the anus which can be felt from the outside.

The lump can increase in size giving rise to a protuberant outgrowth that can even drag heavily as one walks or moves.

Bleeding occurs if there is a rectal prolapse. Fresh blood can be seen in the stools or on the toilet paper.

Alternatively, there may be dark clots that are discharged with the feces.

There may be mucus discharge after passing stools.

The following are the segregated symptoms of arsha roga according to the three doshas:-

Piles due to vata vitiation

  • Piles are hard, rough and blackish in color.
  • They are very painful.
  • Constipation is present.

Piles due to pitta vitiation

  • Piles are tender and reddish in color.
  • They are inflamed and bleeding is persistent.
  • The person feels excessively thirsty.
  • Diarrhea can occur.
  • There may also be slight indications of fever.

Piles due to kapha vitiation

  • These piles are lightly colored.
  • They are larger than the other piles.
  • They are soft and slimy to touch.
  • There is mucus discharge with the stools.


People who are at Risk of Hemorrhoids / Piles (Arsha Roga)

Almost everyone is at risk of piles, since it affects more than half of the world population. The following people are at higher risk:-

People who have varicose veins are at the highest risk of contracting piles.

Pregnant women are at high risk also, they can get piles either during or after the childbirth.

People who are involved in heavy labor, requiring straining the rectal muscles can develop piles.

Piles are rampant among the teenagers who need to sit on hard desks in their schools and colleges for long periods of time.

People who suffer from obesity are at high risk.

Complications due to Hemorrhoids / Piles (Arsha Roga)

Hemorrhoids are generally a mild health problem, which can be easily cured. Even then it is necessary to seek early treatment due to the discomfiture and the pain that the condition causes.

The most crucial complication of piles is when there is blood in the stools. Bleeding piles need to be immediately brought to the attention of a medical practitioner. Even if there are dried blood clots, it may be a serious condition. Blood in the feces could mean cancers of the rectum or the colon; hence this possibility must be verified with the doctor.

Also otherwise, there could be a lot of blood loss each time someone suffering from bleeding piles visits the toilet. In order to stop this loss, it is necessary to get bleeding piles checked with the doctor.

Some piles can cause strangulation of the anal fissure. If the hemorrhoid grows large enough and there is a rectal prolapse, then it could be extremely difficult to defecate. There could be the possibility of waste matter not getting eliminated properly from the rectum, due to which there could be infection and/or toxicosis.

Piles that become hard are called as thrombosed piles. The condition is known as thrombosis. These need to be checked for rectal ulcers or for ulcers in the digestive system. In severe conditions, the blood supply to the piles could be cut off, leading to tissue death within the piles. This gangrenous condition must be urgently checked as the gangrene could poison other regions around the anus.

Prevention of Hemorrhoids / Piles (Arsha Roga)

The following guidelines must be remembered in order to keep hemorrhoids at bay:-

Do not control your natural urge to defecate. As soon as you feel the motion, you must pass it out; or else you will have to exert the rectal area more. This could lead to complications.

Obese people must work at losing their weight. This can be achieved with regular exercise.

Horseback riding must be avoided by people with piles. This can adversely affect the anal area.

Excessive sex must be avoided, especially that which involves the anus.

Anal hygiene is of the utmost importance. Water must be ideally used to keep the anal area clean as toilet paper may not clean the area completely.

It is not advisable to sit on hard surfaces for long periods of time. This could unduly exert the anal area.

Diet for People with Hemorrhoids / Piles (Arsha Roga)

The following dietary guidelines must be followed in order to obtain hemorrhoids relief:-

A person with piles must have lots of water. This facilitates easy expulsion of the wastes from the rectal canal, without subjecting the anus to much pressure.

Green vegetables must be a major component of the diet. These vegetables can provide roughage, which facilitates faster movement of bowels.

Vegetables with astringent and bitter tastes are of much benefit. These will prevent constipations. Helpful vegetables are spinach, okra and yam.

Hemorrhoids relief can be obtained from the itching pain if ghee with warm milk is taken every night.

Barley, wheat and old rice are the carbohydrates that are beneficial for people with piles problems.

The papaya fruit is also very beneficial as it helps in easy digestion of foods.

Pungent, spicy, oily and salty foods must be avoided as they could make the digestion more difficult.

Potatoes, pumpkins and colocasia must not be consumed by people with hemorrhoids. These foods are contra indications to the problem.

This herb is exceptionally beneficial in the case of bleeding piles. It is also beneficial for the piles caused due to vata vitiation.

The herbs mentioned above have been expertly mixed into preparations. These preparations contain just the right proportions of herbs so as not to produce major side effects. The following are some herbs that are effective in the treatment of ascites:-

The following points must be noted:-

  • If there are bleeding piles, then the powder of Nagakeshara given thrice a day in quantities of 500 mg each time has effect. This treatment must be continued for a fortnight.
  • The preparation, Abhayarishta, is very much efficacious in the treatment of hemorrhoid or piles. It contains the herb haritaki which is a traditional cure for arsha roga.

It must be borne in mind that while taking any Ayurvedic preparation, the doctor must be consulted first. Most of the herbs do not have any side-effects (and are indeed a regular part of the diet), but some people may be allergic to certain herbs and may show adverse reactions.

I have piles with excessive bleeding ,and sharp pain througout day. Ayurved and homeopathic are not curing.wat will i do?

Take a cup of boiled milk and bring it to room temperature. Now take a lemon and pour its juice in a seperate cup. Now bring the cup filled with milk close to your lips and pour the lemon into it and consume it immediately. Do it for 7 days. Blood will stop and also pain will reduce. Simultaneously take pilex and triphala guggul.

Fissure in the primary stage should be cured immediately. If over looked from piles.As u know Triphalapowder is antibacterial/fungal/viral it doesn't allow puss formation &curves the wound rapidly. Rectum is full of bacteria n fungus & Will not heel the wound rapidly if over looked. I had small fissure since several mths but it was not trouble some to me.When I came across this topic ,I thought of complications that may be in future. I insurted a small paper roll n stuffed with Triphalapowder 5-6 days, believe the result is satisfied.
